What is the situation? Are you trying to move to another country, or get a job where you might be working with blood or body fluids? Are you applying to give blood or be a sperm donor? Are you participating in some kind of medical study? These are all reasons why someone might understandably want or require info on your HIV status.
If you know that you ought to be clean, there's no reason to be afraid of an HIV test. But ask the doctor WHY. And if he won't tell you, tell him you aren't going to be tested unless he DOES tell you why.
For your own satisfaction -- and safety -- you should get tested anyway. If you know you're HIV-, you have nothing to worry about, and if you know you're HIV+ you can take steps to safeguard your health. Just being HIV+ does not mean you have AIDS and there are drugs to help prevent this now.
